I don't have an actual list but this is what I do. I bought a large cosmetic bag with a large plastic pocket in the center and mesh pockets on each side. When I am home I keep the insulin I am using in one of the mesh pockets and all the other supplies I need to change my site for my insulin pump in the other. When I pack I fill the center pocket with my pump supplies and strips for my meter twice what I expect to need and add it to what is always in there - batteries, three printed out sheet- (what to do when you have very high blood sugar, very low blood sugar and what to do if you need to use a bottle of insulin straight from the refrigerator), ketone testing strips, a glucagon emergency kit, a packet of syringes, and extra meter and the wiring to charge my meter. By keeping most of what I need in the bag all the time it makes easier for me not to forget anything. In another bag I keep my snacks and fast acting glucose tablets.
